On Keeping Up with the Kardashians Season 13 Episode 4, Kim undergoes one final procedure on her uterus in an attempt to carry another baby.

After having complicated pregnancies with both North and Saint, Kim has to have surgery to try and prepare her body to have another child. There are complications after her surgery and she has to have a catheter put in. You can tell she is in a lot of pain, and is desperate to be able to go to the bathroom again. 

After all of that, the surgery is unsuccessful and Kim's doctor tells her she is not going to be able to carry any more kids. It's time for her and Kanye to think about other options like a surrogate. 

Rob also has babies on the brain because in this timeline, Chyna hasn't given birth yet. He is taking steps to get healthy and spend more time with his family to get back to his Armenian roots before he becomes a dad.

Meanwhile, Khloe is preparing to launch her new denim line, Good American. She is nervous because she does not want the line to fail and also worries about being compared to Kylie's successful business ventures. In the end, the jeans sell great and Khloe is thrilled.
